Caregiving Services

How are care plans personalized for each client?

Our care planning process involves three key steps:

  1. Initial Assessment: A licensed nurse conducts a comprehensive evaluation of physical, emotional, and environmental needs
  2. Family Consultation: We meet with family members to understand preferences, routines, and special requests
  3. Custom Plan Development: Our team creates a tailored care plan addressing:
    • Medical needs (medication reminders, vital signs monitoring)
    • Personal care requirements
    • Social and emotional support
    • Home safety considerations

This plan is reviewed quarterly or whenever needs change.

What types of caregivers do you employ?

Our caregiver team includes:

  • Certified Nursing Assistants (CNAs): State-certified professionals trained in clinical care
  • Personal Care Assistants (PCAs): Trained in activities of daily living support
  • Companionship Caregivers: Focused on emotional support and engagement
  • Specialized Dementia Caregivers: Additional training in Alzheimer's and dementia care

All caregivers undergo:

  • Rigorous background checks
  • Drug screening
  • Reference verification
  • Ongoing training (minimum 12 hours annually)
Can care plans be adjusted if needs change?

Absolutely. We understand that care needs evolve, and we offer:

  • Regular Reviews: Scheduled every 90 days
  • On-Demand Assessments: Available whenever you notice changes
  • Emergency Adjustments: For sudden health changes (available 24/7)

To request a care plan review:

  1. Contact your assigned care coordinator
  2. Describe the changes you've observed
  3. Schedule a reassessment visit (typically within 48 hours)

There is never a charge for care plan adjustments.

How do you match caregivers with clients?

Our matching process considers:

  • Care Needs: Matching skills to requirements (e.g., dementia care specialists for Alzheimer's clients)
  • Personality: Ensuring compatible temperaments and interests
  • Availability: Aligning schedules for consistency
  • Preferences: Accommodating special requests (language, gender, etc.)
  • Location: Minimizing travel time for caregivers

We typically introduce 1-2 potential caregivers for your approval before service begins.
